Step #1: Choose the right Florida State Licensing or Regulatory Agency or Board from the options given.
Step #2: Make a payment. After you’re done with the payment, you’ll be taken to the Application Page.
Step #3: Fill in your personal details on the Application Page and click the “Submit” button at the end.
You’ll receive fingerprint cards and/or Drug Testing forms at the address you provided on the Application Form.
Once you get the fingerprint cards, go to your local police station or sheriff’s office to have your fingerprints taken on both cards. Then, send back the completed cards with your fingerprints using the return label provided.
When you get the Drug Testing Chain of Custody Form, visit the nearest LabCorp location for your drug testing.
