Current cost of Medicaid Provider Background Screening at our office is still $80.00 per applicant submission to ORI# EAHCA013Z, which includes (i) fingerprint submissions for Level 2 Screening to FDLE and FBI & (ii) Photo Submission to the Clearinghouse & (iii) 5 year FDLE fingerprint retention costs. Florida Medicaid Provider Enrollment screening was boarded onto  Florida’s Agency for Health Care Administration’s Background Screening Clearinghouse on March 1, 2015 and the old Medicaid Provider Enrollment ORI # FL922013Z was then retired.

All submissions  made under Medicaid Provider Enrollment ORI# EAHCA013Z will provide for prints to be retained with FDLE and thus provide Clearinghouse an ability to require re-screening every 5-years.

Our office located at 4012 SW 18th St, Hollywood, FL 33023. Medicaid Provider Enrollment screening requires two forms of picture ID and credit card or cash payment from applicant. Mobile Fingerprinting Services are Available at your healthcare facilities throughout Miami-Dade, Broward and Palm Beach Counties.

We are fully photo capable to submit your pictures to Background Screening Clearinghouse.
